What is Support Coordination?

Depending on your goals within your NDIS plan you may receive funding for support coordination.

Support Coordination is when someone:

  • Helps you understand your plan
  • Helps you to activate / implement your plan to best achieve your goals
  • Finds the best support for you
  • Helps you build your ability to connect with the supports and community

Our support coordination

Our support coordinators will support you to understand and set up your plan so that you can achieve your goals and build connections with your community to live a more enriched life.

We will focus on you building your skills and capabilities to coordinate the plan yourself so that you can access your supports and your community in the best way that works for you.

We will help you talk to providers about what they offer and how much it will cost out of your plan. We can make sure the service agreements are all completed correctly, so that you are getting what you need.

We will also help you plan for your next review to ensure that you get everything you need to continue to move towards the life that you want.
